Vanna delivers in-person and virtual community-based engagement through an interdisciplinary team of coaches and clinicians. We leverage partnerships with existing community resources to facilitate support networks unique to an individual’s needs and location - connecting them to established psychosocial rehabilitation programs, resources for housing, transportation, etc. and various digital engagement opportunities to address members’ needs.
About the Role
Vanna Health is seeking an experienced, recovery-focused Registered Nurse (RN) who is passionate about community care that addresses all the dimensions of wellbeing and health education. In this role, you will bring passion for serving individuals with mental illness and chronic physical health conditions, helping them access community-based care to improve health outcomes and quality of care for individuals in the Maricopa County community.
While you may be expected to support our general Maricopa County community membership, your role will primarily be facilitating an ongoing pilot initiative evaluating novel care models and outreach strategies for a subset of individuals with complex comorbidities and high health costs. The work emphasizes high-intensity, field-based engagement with the goal of reducing preventable emergency department visits and inpatient utilization. This role will be fully in-person, through our office located in the Central Phoenix area.
Responsibilities
Lead Outreach and Engagement Pilot
Conduct in-person, field-based outreach to enroll high-acuity individuals into our intensive programming
Perform intake assessments, including collection of medical, behavioral, and social history
Provide escalation support to non-clinical care team members for urgent clinical concerns
Perform follow-up visits to reassess clinical and social needs, reinforce care plans and support continuity of care in the least restrictive levels of care
Coordinate with interdisciplinary team members to address identified clinical and social needs
Vanna Care Team Education and Clinical Consultative Support
Conduct interdisciplinary care team meetings to review members’ condition, align on appropriate service coordination as part of the member-centered care plan and promote team collaboration
Educate Vanna care team members to ensure they can accurately and safely monitor for acute or worsening psychiatric symptoms, medication side effects, and physical health needs and can escalate appropriately
Care Coordination
Promote care coordination and communication across healthcare systems
Advise Vanna care team on appropriate services required by members and coordinate services with our clinical partner providers, social service organizations and payers
Direct Clinical Care As Needed
Conduct nursing physical health assessments for members and assist with acute and chronic medical and mental health conditions
Deliver clinical care in the community, including member homes, inclusive of administering long-acting antipsychotic medications and monitoring of vital signs
Adhere to documentation protocols related to medication reconciliation, member appointments, treatment plans, coordination with external providers, and other protocols related to responsibilities
About You
Need to Haves
Minimum of 3 years working with adults with mental illness and substance use disorders in community based settings (inpatient, outpatient, or home health)
Minimum of 1 year of experience working as a medical nurse or working as a nurse in inpatient or outpatient medical settings
Belief that individuals suffering from mental illness can and will recover to achieve life goals when provided the opportunity, support, and community
Ability to work in a team
Willingness to learn how to work with Google, Slack, and Vanna EHR tools
Basic computer and phone skills
A valid driver’s license
Ability to commute into our office in Central Phoenix daily - this is a fully in-person role
Education and License Requirements
Associate’s Degree in Nursing is required
Unencumbered registered nurse (RN) license in the state of Arizona
Nice to Haves
Multistate Licensure
Bachelor’s Degree in Nursing
Bilingual in Spanish and English to ensure we are meeting all members in their most comfortable language
Experience in clinical leadership and program administration
