About The Role

We are seeking a passionate and mission-driven Peer Navigator to provide the best-in-class support to other survivors. At Nema Health, we greatly value the unique and valuable support and coaching Peer Navigators can provide to individuals on their journey toward mental health recovery. We believe these individuals play a crucial role in guiding others through their recovery journeys. Our Peer Navigators provide direct patient support through individual and group mentorship. In these sessions, they empower patients by teaching and supporting the integration of valuable skills into their daily lives. In addition to mentorship, Peer Navigators will manage provider referrals and coordinate between healthcare providers, ensuring that each patient's needs and preferences are met throughout their recovery process. We are currently only considering candidates who reside in the Pacific Standard Time (PST) or Mountain Standard Time (MST) zones.

Your Responsibilities:

  • Provide Nema patients with an excellent experience and seamless care journey by serving as the primary care navigator to assigned patients and guiding them through all 3 phases of care at Nema.
  • Conduct individual peer mentor sessions with patients as they start and recover from a trauma related disorder.
  • Facilitate peer mentor groups on areas of one’s life that traumatic and distressing experiences typically impact and derail.
  • Continuously improve the services that you provide through active participation in training/supervision and expansion of Nema’s peer mentorship offerings.
  • Coordinate care between healthcare providers, and help arrange referrals at discharge from treatment
  • Ensure patient data privacy by accurately maintaining documentation for current patients, in compliance with HIPAA and best practices

Personal Qualities You Bring:

  • You love caring for people, especially trauma survivors, and are constantly examining how you can better improve the care you give.
  • You advocate relentlessly for your patients, flagging any pertinent issues for the rest of the team.
  • You have an infectious passion for supporting trauma treatment and recovery, and can communicate that gift to others.
  • You care about health equity, providing culturally sensitive care and improving health disparities.
  • You are detail oriented, including providing meticulous attention to documentation and patient care details.
  • You thrive in a fast-paced work environment, demonstrating adaptability and a positive attitude toward evolving processes to support scalable growth and operational efficiency.
  • You strive to continuously improve, and know that there is no ceiling to excellence.
  • You recognize your strengths, are aware of your opportunities for growth, and crave constructive feedback.
  • You want to have fun where you work, and to be a part of a team.
