The VP, Care Model Performance will be responsible for designing, deploying, and consistently refining the clinical model at the core of firsthand’s business as well as shaping clinical strategy. Reporting and as a partner to the Chief Clinical Officer (CCO), the VP, Care Model Performance will ensure quality and strong alignment between clinical strategic initiatives developed and implemented, centralized clinical programming, and the care delivered at the market level.
With the primary objective of delivering the best-in-class practices specific to our population served, the VP, Care Model Performance will develop robust care programming informed by evidence as well as data (e.g., claims, internal and external feeds) related to the firsthand population’s utilization patterns, disease burden and unique demonstrated needs, and be accountable for its associated programmatic and organizational outcomes. These programs will bolster firsthand’s interdisciplinary field-based and central support teams in their efforts to engage and build trust with members in their local communities, direct members to the most appropriate settings of care when needed, and identify most-needed social determinant interventions. firsthand seeks an experienced and motivated candidate excited to build an organization from the ground up, with a passion for - and preferred personal experience with - helping one of the country’s most underserved populations.
Key Duties & Responsibilities:
- Reporting to the Chief Clinical Officer, responsible for operationalizing solutions through designing workflows and building technology requirements that are relevant to how our care teams function, accountable for related clinical and business outcomes of select clinical programming for the organization
- Directly supervises a team of clinical strategy managers to support project management, workflow design and implementation planning
- Acts as a primary SME, clinical owner and partner to LT on clinical appropriateness and execution of select care model offerings including but not limited to accurate clinical documentation, quality performance (i.e., HEDIS), benefits support, clinical regulatory considerations, often in collaboration with the CMO, CCO, Operations, Product/Technology, Learning & Development and other key stakeholders
- Support CCO or CMO in communications with the executive team, board of directors, and strategic partners
- Reviewing and continually updating member care standards, programming, and referral systems
- Supporting recruitment and developing training programs for best-in-class providers to deliver high-quality outcomes
- Most importantly, ensuring positive experiences for firsthand’s members, staff, and partners
Required Skills & Abilities:
- Deep understanding of industry-leading practices in population health management in value based payment, including utilization of claims, clinical, and administrative data to inform the prioritization and development of clinical interventions and outcomes tracking
- Applied and working knowledge of continuous quality improvement or other process improvement methodology
- Ability to create a culture of transparency and accountability around team performance and member outcomes in a value based care setting
- Exceptional interpersonal and communication skills, especially in working collaboratively with senior leadership, market leaders, field-based staff, local providers, and strategic partners
- Strong organizational, project management, and problem-solving skills, with a team-focused and continuous improvement orientation
- Experience and comfort working with teams and engaging with members in community settings
- Ability to communicate, inspire, and lead by influence, being flexible and adaptable to changing strategies and needs of firsthand’s organization, its staff, and its members
- Proficiency with Google suite, EHRs, and care management software
Education & Experience:
- Doctorate degree in Medicine, Nursing, Behavioral Health or related field
- Current Board certification in Internal Medicine, Family Medicine, Geriatrics or equivalent
- Additional training in Public Health, Health Administration, Business Administration or related areas preferred
- 3+ years in clinical practice with complex populations, including behavioral health
- 5+ years providing clinical strategic direction and/or supervising clinical or field-based teams at a regional or national level (in a complex care, mental health, and/or value based care setting)
- Experience in a start-up, high-growth, high-change environment preferred
- Track record of leading clinical innovation and successful execution, with a demonstrated ability to positively impact clinical and business outcomes

Vaccination Policy
Employment with firsthand is contingent upon attesting to medical clearance requirements, which include, but may not be limited to: evidence of vaccination for/immunity to COVID-19, Hepatitis B, Influenza, MMR, Chickenpox, Tetanus and Diphtheria. All employees of firsthand are required to receive these vaccinations on a cadence/frequency as advised by the CDC, whereas not otherwise prohibited by state law.
