The Care Navigator, MA serves as the experienced care specialist for Blackbird Health patients. As such, they are primarily responsible for engaging and supporting patients who have begun their care journey with a Blackbird Provider and support families in creating a shared understanding of the precision health care model that Blackbird Provides. Care Navigators are seen as a trusted partner to our patients and providers and as such navigate all established patient care and support a positive experience from the point of intake through to the engagement of treatment services

ESSENTIAL FUNCTIONS:

  • Field inbound calls from established patients ensuring their questions are answered in a timely manner, including directing calls to appropriate team members

  • Schedule and confirm various established patient appointments, including sending reminders and handling rescheduling and cancellation requests

  • Schedule and confirm psychotherapy appointments as identified in the internal referrals system or via provider request

  • Navigate all established patient care and support a positive care experience from the point of intake through to the engagement of treatment services by ensuring patients care questions are answered

    • Support administrative tasks required for patient care

    • Ensure new patient recommendations are sent following each intake appointment, completed recommendations are received and uploaded into the patient chart and the treatment documents are sent following the diagnostic review/ regroup

    • Assist with diagnosis letters, return to school/work forms and other medical forms.

  • Adhere to confidentiality and data protection standards, including HIPAA regulations, to ensure patient privacy and secure handling of sensitive information.

  • Attend team meetings to support organizational success and participate in ongoing training required by organization to ensure high quality services are delivered

  • Able to independently problem solve and organize time to consistently achieve productivity and quality standards for role

  • Use new technologies as directed by leadership (e.g., Luma communications platform, EHR, internal tracking systems)

  • Support in person office needs - e.g., receive mail, packages, restock supplies, tidy up toys, sanitize shared spaces, ensure bathroom cleanliness, greet patients and answer questions)

  • Hybrid work model with up to 3 days per week in the Fort Washington office with the ability to cover our King of Prussia or Exton offices as needed. The remaining work hours can be managed remotely.

REQUIREMENTS:

  • Certified Medical Assistant or degree in Healthcare Administration or related field

  • Minimum 1 year of experience working in a medical office or healthcare setting

  • Experience working in an electronic health record or patient engagement platform required

  • Telephonic outreach experience

  • Customer service experience

  • Computer skills; Windows, Microsoft, Excel, Google docs, Google Meet

  • Passion for helping others

  • Works efficiently, and is able to maintain the daily/monthly volume of work consistently

  • Ability to connect with and engage people from diverse backgrounds, experiences, cultures

KNOWLEDGE AND SKILLS:

  • Computer Skills – Must be PC literate with proficiency typing, navigating email, calendaring and word processing.
